Issuance of health certificate

Dear Shinagawa Campus students,

It is now possible to issue a health certificate for 2020 using an automatic certificate issuing machine.
After taking the regular health checkup for students in September,Students who are subject to re-examination and students with chronic illnesses will not be issued a certificate from the automatic issuing machine, so please check at the Health Center.
*Students who cannot issue a certificate will be prompted with the message "Insufficient information."
(As a general rule, the certificate will be issued at the Health Service Center two days after the application date.)
