Issuance of health certificate

Dear Etchujima Campus students,

It is now possible to issue a health certificate for 2022 using an automatic certificate issuing machine.
After undergoing the regular health checkup for students in April,Students who are subject to re-examination, students with chronic illnesses, and students who have not responded to the online questionnaire will not be issued a certificate from the automatic issuing machine, so please check at the Health Center.
*Students who cannot issue a certificate will be prompted with the message "Insufficient information."
(As a general rule, the certificate will be issued at the Health Service Center two days after the application date.)
