Regarding advance submissions

①Tokyo University of Marine Science and Technology medical examination form
Please fill out the medical questionnaire on the front and back of the cover. If you have any questions regarding vaccinations, please refer to your maternal and child health handbook and fill in the information accurately.
Past medical history and allergies will be used in various training sessions, including ship training, so if you have any questions, please consult with your parents or your doctor and fill in the details.

② Electrocardiogram questionnaire (first year undergraduate students only)
Please fill in the questionnaire regarding heart disease and blood pressure within the bold frame.
*The above documents will be used for the April health checkup. There is no need to undergo a medical examination in advance.

Investigation of medical history of infectious diseases such as measles and vaccination history

Regarding question item 2 of the Tokyo University of Marine Science and Technology medical examination form: "Please fill in information about vaccinations",
Be sure to check your maternal and child health handbook, etc. to confirm whether you have had any infectious diseases and whether you have received vaccinations.
