Regarding issuance of health examination certificate

Dear Shinagawa Campus students,

Health examination certificates for 2024 can now be issued using automatic certificate issuing machines.
After undergoing the regular health checkup for students in April,If you are subject to re-examination, if you are currently undergoing treatment such as outpatients, if you have had a major illness such as surgery, or if you have not responded to the online questionnaire, issue a certificate from the automatic issuing machine. cannot be issued.In that case, please check at the Health Administration Center.
*If it cannot be issued, the message "Necessary information is insufficient." will be displayed.
(As a general rule, the certificate will be issued at the Health Service Center two days after the application date (on a weekday).)
